Thank you for considering a referral to Creating Ethical Boundaries. Please download the CEB Board Referral Package in your choice of pdf or editable word formats. Please complete the submission documents on the appropriate forms, and include the requested information for our review. Once complete, simply mail or fax the completed forms along with any additional information or documentation.
In addition to the form, what does CEB need to process the referral? We request any supporting materials the board is permitted to send, including but not limited to a copy of the final order of disciplinary action.
How will CEB maintain contact with the referring board? We recommend the referring board identify a primary point of contact. This person will receive updates from the Creating Ethical Boundaries staff and will receive notice when the professional has completed the program. An alternative contact is also recommended should the primary contact not be available.
How long does it take to initiate services? In the Order for Disciplinary Action, we recommend the professional be given a deadline by which to initiate services. Generally, a professional can be expected to complete the Creating Ethical Boundaries tutorial in less than a year (see PROCESS). While the length of the program is variable, a professional can begin receiving services as soon as the enrollment process is completed. Once a professional contacts Creating Ethical Boundaries, within 3-5 business days the professional can be fully engaged in services.
What is the basis for CEB's recommendations? The program administrators use a propriety evaluation system as an internal metric that examines the nature of the violation, public impact, the severity of the violation, and the number of disciplinary actions (current violation and previous). The data is then correlated, and on a case by case basis with an interview, the CEB administrator can recommend the most appropriate, effective and beneficial directive to each licensee. The CEB Program provides three distinct packages for compliance relative to the type of violation.
